Chapter 1Buried PenisWhat is Buried Penis My sister-in-law brought her over-weight son at the age of 12 years because his penis could not be seen clearly to see a well-known Pediatrician Professor.Unfortunately the Professor was getting a bit deaf in the ears.So when my sister-in-law said that he had a small penis, she had to shout it loudly to him.In return he shouted ‘fat” at her.At first she could not understand, then she realized that he was referring to his fat covering his penis.She then realized that the fat was burying his penis so that it could not be seen clearly.So the obvious solution was to lose some fat so that the penis is more obvious.The Professor did not recommend any surgery.She reduced his food intake to reduce his obesity.Later on in National Service, he was forced to lose more weight during his training.There was no issue with his penis since then.Buried penis is a medical disorder that has a penis of normal size that lacks a proper sheath of skin It is located beneath the skin of the abdomen, thigh, or scrotum.This disorder is more frequent in children, usually in neonates or over-weight pre-pubertal boys;It can also be observed in adults and has been seen in both circumcised and uncircumcised individuals.Patients who are marginal may not be detected until adulthood, when higher fat deposition accentuates the problem.There are several ways of classifying buried penis, although none has been universally adopted in the literature.Typically, differentiation among the terms is:1. Concealed (before circumcision),2. Trapped (cicatricial or scarred after circumcision),3. Buried (associated with adolescence and obesity).In most congenital pediatric cases, the buried penis is will get well by itself.In adults who are not treated, the patient tends to worsen as the abdominal pannus continues to grow.Buried penis is a true congenital disorder.Some doctors also refer to this disorder as a hidden or concealed penis.Trapped penis is a disorder in which the penis becomes inconspicuous secondary to a cicatricial scar, normally after overzealous circumcision.PathophysiologyThe penis is covered in a sheath of skin.Normally, the skin is evenly spread around the entire penis, down to the scrotum.However, some boys are born with skin that unevenly covers the penis.There is less skin on the portion of the penis closest to the scrotum.The penis also is hidden within the fat inside the lower part of the abdomen.This is called a buried penis.Doctors also call it hidden or concealed penis.Boys are born with a buried penis (congenital).There are many causes which include: 1. Problems with the skin and fat layers surrounding the penis of a developing fetus.2. Too much fat on top of pubic bone, which is common in young babies.3.
